Formaldehyde is classified as a probable carcinogen, or cancer-causing substance

BAY ST. LOUIS, MISS. - The anguish of Hurricane Katrina should have ended for Gina Bouffanie and her daughter when they left their FEMA trailer. But with each hospital visit and each labored breath her child takes, the young mother fears it has just begun.

"It's just the sickness. I can't get rid of it. It just keeps coming back," said Bouffanie, 27, who was pregnant with her now 15-month-old daughter, Lexi, while living in the trailer. "I'm just like, `Oh God, I wish like this would stop.' If I had known it would get her sick, I wouldn't have stayed in the trailer for so long."

The girl, diagnosed with severe asthma, must inhale medicine from a breathing device.

Doctors cannot conclusively link her asthma to the trailer. But they fear she is among tens of thousands of youngsters who may face lifelong health problems because the temporary housing supplied by the Federal Emergency Management Agency contained formaldehyde fumes up to five times the safe level.

The chemical, used in interior glue, was detected in many of the 143,000 trailers sent to the Gulf Coast in 2006. But a push to get residents out of them, spearheaded by FEMA and the Centers for Disease Control and Prevention, did not begin until this past February.

Members of Congress and CDC insiders say the agencies' delay in recognizing the danger is being compounded by studies that will be virtually useless and the lack of a plan to treat children as they grow.

"It's tragic that when people most need the protection, they are actually going from one disaster to a health disaster that might be considered worse," said Christopher De Rosa, assistant director for toxicology and risk assessment at the federal Agency for Toxic Substances and Disease Registry, an arm of the CDC. "Given the longer-term implications of exposure that went on for a significant period of time, people should be followed through time for possible effects."

Formaldehyde is classified as a probable carcinogen, or cancer-causing substance, by the Environmental Protection Agency. There is no way to measure formaldelhyde in the bloodstream. Respiratory problems are an early sign of exposure.

Young children are at particular risk. Thousands who lived in trailers will be in the prime of life in the 10 to 15 years doctors believe it takes cancer to develop.

FEMA and CDC reports so far have drawn criticism.

A CDC study released May 8 examined records of 144 Mississippi children, some of whom lived in trailers and others who did not. But the study was confined to children who had at least one doctor's visit for respiratory illness before Katrina. It was largely inconclusive, finding children who went to doctors before the August 2005 storm were still visiting them two years after.

A bigger, five-year CDC study will include up to 5,000 children in Louisiana, Mississippi, Alabama and Texas, and CDC officials said it should begin next year. But members of Congress point to the decade or longer it could take for cancer to develop and say a five-year look is inadequate.

"Monitoring the health of a few thousand children over the course of a few years is a step in the right direction, but we need commitment," said Rep. Bennie Thompson, D-Miss.

Thompson has introduced legislation to force FEMA and CDC to provide health exams for trailer residents who believe formaldehyde made them ill. The bill is similar to $108 million legislation for workers who labored at the World Trade Center site.

Arch Carson, professor of occupational medicine at the University of Texas Health Science Center in Houston, said preliminary exams alone for trailer residents could cost more than the trade center bill. But he said class-action lawsuits over the formaldehyde — at least one has been filed — could be even more expensive, costing many billions of dollars.

"It would be best for the government to get its act together now," Carson said.

More than 22,000 FEMA trailers and mobile homes are still being used in Mississippi and Louisiana.

FEMA and the CDC say they will create a registry of those who stayed in trailers for possible future study. But they admit that the task of keeping track of everyone is made difficult by the rush to get families into other housing.

The parents of McKenzie Whitney, a 1-year-old girl with wavy auburn hair, are running low on money and options for caring for the sick girl.

Born into a FEMA trailer, McKenzie was out of the dwelling in August 2007 after a 10-month stay. Her mother, Kacey Whitney, 22, a housekeeper, and her father, Kevin Whitney, 30, a maintenance man, juggle the pressures of post-hurricane life with tending to the child.

"Sunday night when I was going to work, as I was walking up to the front door, she just threw up. She had a fever. We went to the hospital and they wound up keeping her overnight," the girl's mother said. "She's always had a cold, always."

Like Lexi, McKenzie is treated with a nebulizer, a boxy breathing machine that turns medication into mist. It is prescribed to patients with moderate to severe symptoms, and requires children to inhale for 20 minutes.

Dr. Shama Shakir, a Bay St. Louis pediatrician who treats Lexi and Kacey at the Coastal Family Health Center, said that before the storm she prescribed nebulizers about twice weekly. Lately, she is doing so up to 12 times a week.

"You give them the most potent steroids, the most potent antibiotics, and still they have the symptoms," Shakir said. "I worry about what will become of these children long-term."

Deven Galloway, 27, lived in a FEMA trailer in Bay St. Louis for seven months with 4-year-old son DeReion. The boy uses a nebulizer for asthma.

"One day he was like, `I'm going to take more so I can go ahead and be finished for a long time,'" said his mother. "I had to tell him it didn't work that way."

What is A-1ATD?

Alpha-1 antitrypsin is a protein that is made in the liver. The liver releases this protein into the bloodstream. Alpha-1 antitrypsin protects the lungs so they can work normally. Without enough alpha-1 antitrypsin, the lungs can be damaged, and this damage may make breathing difficult. In addition, liver damage (hepatitis, cirrhosis) can occur in both children and adults. Alpha-1 antitrypsin deficiency is an inherited (passed down from parents) disorder that causes low levels of, or no alpha-1 antitrypsin in the blood.

How do normal lungs work?

Air usually enters the nose and mouth and goes down the air tube (trachea) to two main air passages (bronchi). These passages allow air to go into the right and left lung.

Each bronchus branches out into grape-like air sacs called alveoli. Through the alveoli, oxygen enters the bloodstream during breathing in (inspiration), and carbon dioxide, a waste product, leaves the body during breathing out (expiration).

White blood cells normally found in our bodies help protect us from infection. But white blood cells also release an enzyme, called neutrophil elastase, that can damage the lungs. In normal lungs, alpha-1 antitrypsin protects the lungs from the harmful effects of neutrophil elastase.

How is the influenza virus spread?
The flu spreads through thee major modes of transmission: large droplets and aerosols (small airborne particles) – spread mainly through coughing or sneezing and direct contact with secretions (on surfaces for example).

What can an air cleaner do?


Large-droplet transmission is the main way in which the influenza virus is acquired. Coughing or sneezing generates a substantial quantity of particles, a large number of which are less than 5 microns in diameter. Small particles are also a main concern because particles expelled by coughing or sneezing rapidly shrink in size as they evaporate.

Air pollution seen raising preterm birth risk


NEW YORK (Reuters Health) - A study conducted in Los Angeles County and published today shows the harmful effects traffic-related air pollution can have on pregnant women.

The data suggest that women who live in areas with high carbon monoxide or fine particle levels - pollution caused mainly by motor vehicle traffic -- are roughly 10 to 25 percent more likely to suffer preterm birth (delivery before 37 weeks of pregnancy), compared with women who live in less polluted areas.

This is especially true for women who breathe polluted air during the first 3 months of pregnancy or during the last months and weeks before delivery.

Importantly, researchers report in the American Journal of Epidemiology, the association between air pollution and increased risk of preterm birth persists after accounting for other factors that might influence preterm birth risk such as smoking, exposure to second-hand smoke and alcohol use.

"Air pollution in Los Angeles County remains a major public health problem affecting everybody, particularly pregnant women," Dr. Beate Ritz from the School of Public Health at the University of California, Los Angeles noted in comments to Reuters Health.

Ritz and colleagues collected detailed information for more than 2,500 women who gave birth in Los Angeles County in 2003. By conducting one-on-one interviews with the women, the researchers were able to separate the air pollution risk from other preterm birth risk factors.

"Our research group had previously reported on the effect of carbon monoxide and fine particles, but because we relied on birth certificates, we did not have detailed information about other risk factors that some people suspected might bias our research findings," Beate explained.

This new study, she said, "helps confirm the results we reported previously - that air pollution mainly caused by vehicle traffic increases the risk of preterm birth even when we take other risk factors into account."

Manila Breathing Easier as ADB Releases Last Tranche for Clean Air Project

MANILA, PHILIPPINES (15 December 2003) - ADB has released the last US$100 million for the Metro Manila Air Quality Improvement Sector Development Program being implemented by the Department of Environment and Natural Resources (DENR) after achieving significant reductions in lead and sulfur dioxide levels.

A recent study showed improvements in blood lead levels among children in Metro Manila compared to 2000 when the unleaded gasoline policy was first implemented. Lead poisoning reduces intelligence quotients, especially in children. At its worst, the level of lead exposure in Metro Manila can cause an average loss of five IQ points in children as well as heart disease and premature death.

"Despite many challenges, the Government has made significant progress with the program and has achieved the benchmark reforms it set for the release of this second and last tranche," notes Yue-Lang Feng, the senior ADB officer in charge of the program.

"The Government will continue its efforts as air quality management and improvement is a long-term process."

Immediate effects of short-term exposure to forest fire smoke include:

  • Sore eyes

Note: The mixture of particles, liquids, and gaseous compounds found in smoke from wild land fires is very complex, and include compounds that can irritate and even injure the tissues of your mouth, nose, throat, and lungs. During past fires in Florida, an increase in emergency department visits was seen for asthma, acute worsening of chronic bronchitis, eye irritation, chest pains, shortness of breath, and wheezing.

State bans in-home ozone air purifiers, citing health risks
The California Air Resources Board says the regulation, which takes effect in 2009, is the first of its kind in the nation.


(Los Angeles Times) The California Air Resources Board has banned popular in-home ozone air purifiers, saying studies have found that they can worsen conditions such as asthma that marketers claim they help to prevent.

The regulation, which the board said is the first of its kind in the nation, will require testing and certification of all types of air purifiers. Any that emit more than a tiny amount of ozone will have to be pulled from the California market.

An estimated 2% of the state's households have one of the so-called ozone air purifiers, according to air board staff research, and the staff estimated that more than 500,000 people had been exposed to levels of ozone above federally recognized health standards as a result. More than 2 million California residents have some sort of air purifier, and other types can be safe and effective, the air board staff said.

"This is a landmark decision," said Mary Nichols, chairwoman of the Air Resources Board.

"State government needed to set up [its] own standards on air purifiers because many [marketers] indeed are deceiving the public," said former Assemblywoman Fran Pavley, who sponsored a law requiring the board to rein in so-called ozone generators. "There are reports of ozone being generated in someone's living room . . . at levels equivalent to having a Stage 1 smog alert right in your own house."

The new regulation, which takes effect in 2009, will exempt industrial and commercial uses of ozone generators, as long as people are not present.
